Diagnosis – January/Feb 2010 (DCIS – Stage 2 Breast Cancer)

On the 2nd week of December 2009 (before Christmas), Nanay discovered a big lump on her right breast (accidentally).
she did not feel any pain or any symptom that will conclude this is cancerous. However, that cloud of fear lingered over our heads during the Christmas and New Year Holidays.

Ultrasound Breasts
Both breasts exhibit normal internal stromal echo pattern and structure
There is no focal stromal distortion
Solid nodules are seen as follows:
1. 2-3 o’clock: Two benign nodules measuring 6mm and 4mm respectively
2. 3 o’clocl: 4mm void, well-deifned but tall nodule, Inderterminate
3. 6 o’clock: 9mm, slightly irregular nodule with mild posterior acoustic shadowing. Indeterminate
4. 11 o’clock, periarreolar: 31mm heterogenous lesion with well-defined but very irregular margins and increased vascularity. This is suspicious.
1. 12 o’clock: 2mm slightly tall ovoid leion. Inderterminate
2. 2o’clock: 3mm, slightly tall ovoid lesion. Indeterminate
3. 3 o’clock: 5mm benigh
4. 6 o’clock: 4mm,slightly tall ovoid lesion. Indeterminate
5. 9 o’clock 4mm, rounded lesion. Indeterminate
6. 10 o’clock: two benign nodules measuirng 4mm and 5mm respectively
A mildly enlarged right axillary lymph node is present.
A few cysts measuring up to 7mm are seen in the right breast.
Solid nodules seen in both breasts as described above. Note the presence of suspicious lesion in the right breast at the 11 o’clock position with right axillary lymphadenopathy.
There is an ill-defined opacity seen in the right upper outer quadrant at the periarreolar region. There are associated pleomorphic microcalcifications in this area. There are also isolated foci of benign type calcification in both breasts. No suspicious left breast leasion is seen.
There is no nipple retraction but the right axillary nodes appear larger and denser than the left
The findings in the right breast can be classified as BIRADS 5 category. There is also suggestion of ipsilateral axillay node involvement.
BIRADS Category 5 — Malignant (RIght breast Tumour)
There are masses with an appearance of cancer. A biopsy is recommended to make an accurate diagnosis.
Under local anesthesia a core needle biopsy was conducted on the 4cm tumour on my right breast
Diagnosis: Ductal Carcinoma in SITU (DICS) — a type of breast cancer.
In terms of tumuor size, the Breast Cancer is at Stage 2
On the same week we went to National Cancer Centre Singapore (SGH) for a 2nd opinion, the diagnosis, findings and recommended treatment is the same, a RADICAL MASECTOMY with Sentinel Lymph Node Biopsy. (which means that a 15-25 lymph nodes under the armpit will be removed.
